FHA Streamline Refinance

I am very new to the business (about a month) and have a few questions about the FHA Streamline Refi. If someone could provide me with a sort of checklist of information that is needed from the customer I would greatly appreciate it. Also if anyone has some worksheets they find useful. Any and all information would be GREATLY appreciated. Thanks in advance!
